
March 1st, 2012 Our AF Spring Grant Cycle has begun.
Applications from qualified 501(c) 3 organizations accepted through April 20, 2012.

After you have determined that your organization is eligible, please follow the steps outlined below to submit your application.
NOTE: AF does not have a formal online application form to fill out. Your organization will use AF's guidelines to construct your application.
Grant applications must include all of the following information:
1. A one-page cover page, including:
Applicant organization’s name, name of person submitting proposal, organization’s address, telephone and facsimile numbers and e-mail address (if available), exact dollar amount requested, total operating budget, and total project budget.
2. Brief narrative on purpose and history of the organization and project, including:
Mission statement, recent accomplishments, the need being addressed, and summary of project for which funds are requested. (2 pages maximum)
3. Expected project outcomes with timeline and plan to monitor and evaluate project success. (1 page maximum)
4. Brief description of key staff and/or volunteers and a list of Board of Directors and their affiliations.
5. Letters of intent from other organizations collaborating in proposed project, if applicable.
6. Evidence of past organization successes and accomplishments as appropriate.
7. Financial information, including: Organizational budget, event or project budget, and most recent year-end financial statements (audited if available).
8. A copy of the organization’s IRS 501(c)(3) determination letter. We fund only public charities as defined under sections 509(a)(1), 509(a)(2), or 509(a)(3) of the IRS Tax Code.
9. A copy of the organization’s official non-discrimination policy including sexual orientation.
10. Special notice to 501(c)(3) organizations involved in lobbying activities: Please include a copy of the organization’s 501(h) election form.
11. Special notice to local chapters of national organizations: Please include an overview of how your local organization and the national office will work together
The Adam Foundation requires submission of final reports. Interviews may be requested by the Adam Foundation.
